Covering letter from the applicant addressed to Philippines Consulate, Mumbai with proper entry & exit date.
Photo 50x50mm with 80% face coverage.
Hotel confirmation with payment receipt.
3 Year IT returns.
3 months current bank statement with atleast closing balance of INR 30,000
3 month salary slip & leave letter in original with proper dates.
Credit / Debit card copy or Forex receipt copy for 500 USD

Indian Nationals may be granted visa – free entry for as initial stay of not exceeding 14 days, provided they possess the following:
Either a valid US, JApanese, Australian, Canadian, Schengen, Singapore and UK Visas or permanent residents permit
A national Passport are valid at least 6 months, beyond the contemplated stay;
Return or onward ticket to the next country of destination;
No Derogatory record with the Bureau of Immigration (BI), the national intelligence Coordinating Agency (NICA), and the International Police (Interpol).
DD has to be made in favour of “Consulate General Republic of the Philippines”.
